Click on Edit

Click on Design
You will be presented with a screen. On the right you will see your form. On the left you will see your customization options. Here you can change the form design.

Click on Welcome page
Here you can change the copy on your welcome page

Click on Response page
Here you can change the responses you collect from customers

Click on Customer details page
Here you can choose the details you ask customers

Click on Save changes
Continue working through to customize your form. Then click on Save changes when you're happy.
