Find steps below for submitted sessions:

  1. Uploading contracts or certificates of insurance from 3rd-party groups.

  2. Uploading other documents to the Documents tab for your records or to assist the Office of Youth Protection’s review process.

1. Contracts & Certificates of Insurance

3rd Party Organization options are only available if the session submitter indicated that a 3rd party group would be involved when they registered the session. If you need to add a contract and certificate of insurance and you don't see the Add 3rd Party Organization button, click Request Edits... to submit a request to the Office of Youth Protection.

Upload the organization's certificate of insurance

This step requires a certificate of insurance from the 3rd party, listing OSU as additionally insured—not OSU's certificate.

Insurance and Risk Management has provided guidance for certificates of insurance. Be sure to scroll to the When Oregon State University Requires a Certificate of Insurance heading.
