1. The following instructions assume you have already enrolled a person into the OneHome_CES program in HMIS and completed the relevant CE Assessment (currently VI-SPDATs) for them.

2. After saving the VI-SPDAT, you will now see multiple Community Queue options to select from. You will ALWAYS want to select the 'OneHome Community Queue (Assessors Use This)' option. It is the first one listed. Please do not select any other option.

Note: adding clients to one of the 'MDHI Staff Only' options will result in our team reaching out to you

3. After selecting the 'OneHome Community Queue (Assessors Use This)' option, click on 'REFER DIRECTLY TO COMMUNITY QUEUE(S)'

5. To submit the referral to the CQ, click 'SEND REFERRAL'

6. Please note: if you accidentally forget to refer the client to the OneHome Community Queue after saving their VI-SPDAT, you can navigate back to their Assessments tab within their enrollment and click 'Eligibility' next to their VI-SPDAT score
