For Program Name, please use the following naming convention Institution - <clinical> or <combined clinical + research>.

Select "Years of Training" from the drop-down.

Please select the minimum duration that your program can guarantee. If your program has an option to extend the duration of the fellowship, please add that information to the description field.

Add the number of "Positions Available."

Programs can add additional available positions prior to the match list due date.

Add a description of the fellowship position for applicants to review.

This can also be added or edited later. Keep description brief, as applicants will also have a link to the program's website.

Select "Program Role" to include your director and associate program contacts and fill out contact details.

Once all the fields are complete, please "Submit" your program registration.

Once approved, the program will:

  • Receive confirmation with the program’s 4-digit SF Match ID

  • Each user listed on the form will receive individual emails to setup user accounts (temporary password provided on email expiries within 7 days. If temporary password has expired, an existing user can login and trigger a new one or they can contact help@sfmatch.org/415-447-0350.)

  • Receive Invoice and instructions to pay for registration ($500)
