Prepare Tax Returns - Generating and Approving 1095C's

The purpose of this training is to walk you through the steps of reviewing the quality control documents and generating, reviewing and approving your 1095C forms.

Go to ACA Track via login.myacatrack.com and log in using your login credentials.

Click on Prepare Tax Returns

Only start with this training after you have uploaded your final data and you receive the message of "Data Upload Complete".

Review Batch Uploads

This section is referencing the file that you uploaded in Data Management. **If your plans are self-funded or level-funded, you should have a dependent data upload as well. If your plans are self-funded or level-funded and the Dependent Data load section is blank, return to Data Management to upload your dependent file until is it error-free.

View Batch Detail

To proceed you will need to view a batch detail report which is a listing of all employees from your data that will receive a 1095C. Click on "View Batch Detail" to download and review the list of employees.

Open the report in your downloads. Ensure all Employees and/or Dependents are accounted for with accurate information. If anyone is missing, return to Data Management, delete your file and reload the file with corrections. Alternatively, reach out to your Reporting Specialist.

Click on Click to Confirm Accuracy of Names and Addresses

If the batch detail is correct, click on "Click to Confirm Accuracy of Names and Addresses"

OPTIONAL STEP - ONLY IF YOU HAVE ELECTED STATE FILING SERVICES.

If you have elected State Filing services, you will see Step 3. Review the State filing column in the batch detail report and click to confirm if approved. **Note that only CA, NJ, DC and RI are processed within ACA Track. If you have elected MA 1099-HC state reporting, that is handled separately outside of the ACA Track System.

Approved Batch Detail

Once verified, you should see the green button indicating that you have approved the Batch Detail Report. If the button is still red, the batch detail has not been viewed or verified.

Click on 2. Audit Quality Review

The Audit Quality Review step is an additional step to further analyze your data for potential problems. A red exclamation indicates this section has not been completed.

View and Approve Client Verification Report (CVR)

Click on the blue button "Click to View Reports". This will download the CVR which is a confirmation of the information setup for your organization, including EIN's and plans.

Download and View CVR

Below is a screenshot of the first page of the CVR, which explains what each section is showcasing in the report. Be sure to review each page for accuracy. If anything is incorrect, reach out to your Reporting Specialist for assistance. Keep in mind, some information on this report could be historical from a previous reporting year (such as plan ID's and mapping codes). Verify CVR

If accurate, select "Click to Verify Report". You cannot proceed until the report is verified. If the report is not accurate, or you have concerns, reach out to your Reporting Specialist.

Download and view the Data Quality Report (DQR)

Click on "Click to Generate New Report" which will download the Data Quality Report. This further analyzing your data against the information in our system, such as plan information rules and company setup. Once clicked, you will see the 'Report Generation in Progress' message until the report downloads.

Review the DQR

If the information is not accurate or if you have a question, reach out to your Reporting Specialist for assistance.

Click on "Please Generate my 1095 Forms"

If the required reports have been "verified" you can now generate your 1095C forms. If this button is grayed, then a step has been missed.

Type "Confirm"

Status: Your 1095 Forms Are Being Generated

Your 1095C's are in the process of being generated and may take several minutes. You will receive an email notification informing you that the 1095C forms are ready for review and approval

After you have received the email notification stating that your 1095C forms are ready for review, navigate to Step 3: Review and Approve 1095Cs

Click on the blue 'view' text to download your 1095C forms in PDF format. You can then review them for accuracy.

This is a sample 1095C form. Be sure to review all fields for accuracy. For details on various codes, please visit Understanding the 1095c codes If you believe a 1095C form is incorrect, please reach out to your Reporting Specialist for assistance.

Approve Forms

If the forms are accurate, click on the 'Approve' button. If the forms are incorrect, click on "Reject" and reach out to your Reporting Specialist for assistance.

Print and Mail 1095C Forms

Once you have clicked on "Approve" then you'll see a new button "Send 1095C's to the Printer". By clicking this button, you are confirming that your 1095C's are ready to be sent to the ACA Track printer for printing and mailing. Forms are mailed within 7-10 business days.
