Navigate to the "Form Builder" option via your menu bar

Click on Create New Form

Give a title to your form

Click on Add Content to start adding questions
If you want to set a deadline for your form, be sure to do that too!

Enter your question in the "Content/Question Label" box

Choose your desired response type under "Content Type"
E.g., Short answer, Long answer, etc.

Add any placeholder/example text

If you want to make the question required, toggle that option
If you do not want to require a response, you can leave this unchecked. Additionally, you can click the trash can to delete the question.

You can also add additional questions.

For example, you can add Multiple Choice

It is easy to add many options/question styles to your form.

Once you are done be sure to save your form!
