12. 12. List any dietary restrictions for this representative. If none, leave blank.

    

13. 13. Type the name this representative would like on their name badge

    

14. 14. Type the first year your company showed at a SWACUHO Annual Conference. If unknown, put N/A.

    

15. 15. To add one or more additional representatives, click Add guest

    

16. 16. Type additional representative's first name and all other required fields

    

17. 17. Click on Done

    

18. 18. Click on Next

    

19. 19. If finished, Click on Invoice me to pay via check OR Pay online to pay via card on payment form.
