Visit excite.extension.org and under "EXCITE Resources," click on "Implementation Phase Reporting."

On the left-hand side of the screen, you will see the four forms. Click on the form you wish to complete. You may also share a link to any of these forms with anyone you need to complete them. 

Complete the form and click submit. 

NOTE: The Quarterly Financial Reporting, Invoice Submission Form, Change of Statement of Work Form, and Change of PI Form do NOT require any login access to complete. We realize that forms like the Quarterly Financial Reporting may be completed by your university finance office. Anybody can complete these forms without requiring any login access.
