Log in to your EHSA account and select the "EHS Assessment" icon.

In the "User/PI" field select the account owner you wish to create an LHAT for.

Click on the "+Browse New Forms" button

A new window will pop-up showing a list of available assessments. Select the Lab Hazard Assessment by clicking the green "Start" button in the left-most column near the "Lab Hazard Assessment" option.

Make sure you have the correct account owner listed in the "PI" field and click on "Create Assessment."

You will be navigated to a page showing all current data we have for the selected account owner. This includes personnel and location lists. In order to add a worker to the LHAT, click on the "Find Worker" button.

A select worker window will appear where you can search our database for the appropriate individual. We recommend searching by last name or by their UID. Once you have entered your search information into the "Find Worker" field click on the "Search" button to the right.

The table will populate with all matches in our system. Select the worker of interest by clicking on the "Select" button to the left of their name. Close the window when you are done adding personnel.

To remove a worker from your LHAT simply click the "x" button on their name in the Attached Workers table.

To add a location to your LHAT, navigate towards the bottom of the form and click on the down arrow near the Building field.

Selected the building you wish to add in the drop-down menu. You can also type the name of the building into the Building field to filter for your selection. Once you have found your building click on it to load the room options.

Select the room you wish to add by clicking on the green arrow near the room of interest in the table.

To remove a location from the LHAT simply click on the x button near the room you wish to remove.

Once you are done updating workers and locations to your LHAT, click the "Next" button.

You will now be navigated to the Assessment Questions. Please read each question carefully and answer to the best of your ability.

Answering "Yes" to some questions will prompt additional questions related to where certain materials will be handled to populate.

You may select rooms individually by checking the box next to each location.

You may also select all locations by clicking on the "Select All" button.

Once you have completed the Assessment Questions click on the "General Information" tab on the left-hand ribbon.

Select your department using the drop-down menu.

Please provide any additional information related to your division or institute in the "Division/Institute" field.

Lastly, please provide us a brief research summary by clicking on the "Research Summary" tab in the left-hand ribbon entering your summary in the text box on the right. We use this information to help labs obtain access to appropriate safety resources.
