Click on Health Authorizations
In the General Forms Tab

Review and Complete the Form

Insert your digital signature

After clicking OK you will be brought to the next page- Health Information Update

After completing the form, Click on Submit

After clicking OK you will be brought to the next page- Permissions and Agreements

Click on the included hyperlinks to review policies and procedures, throughout the form.

Select the acknowledgements, and your other directory preferences, then complete the form.

Click on Submit

Click on OK
After clicking OK you will be brought to the final page- Student Information Updates

Click the hyperlinks throughout the form to review notices and information, then select acknowledgements.

Check I hereby certify that all information given on this enrollment form is true and correct.

Check I consent to electronic signatures

Click on Submit
This form can be saved for other students. Next to submit, click on the small arrow pointing up, then select "Submit for Family" this will allow you to choose your other students that you would like to submit this form for.
