Submit OSU Criminal History Check Authorization

OSU Youth Program Registry

1. 1. Select OSU Login or Non-OSU Login and enter your login credentials

   Choose OSU Login if you are using an ONID account.

   

2. 2. Click on Begin

   The Action Required banner appears when:

   • You are assigned to a session, AND

   • You have not completed a criminal history check in the last 24 months, OR

   • Your criminal history check is expiring in the next 60 days.

   

1. 3. Complete the Submitter Details section

   Enter your date of birth, e-mail addresses, and legal name. Double check for typos!

   

2. 4. Add a phone number

   

3. 5. Enter your home address

   

4. 6. Upload a copy of an official photo ID showing your name (e.g., Driver's License, Passport, Student ID)

   

1. 7. Indicate whether or not you have lived outside of Oregon in the last 7 years

   

1. 8. Click on Next >

   

2. 9. Enter your full name

   

3. 10. Click on Next >

   

4. 11. Review the form for accuracy and click Submit Form
