Locate the "User/PI" drop-down menu.

From the drop-down menu near "User/PI," select your department.

Click on the row with your EAP/IIPP Assessment. The row should become highlighted in red.

Click on Assessment Reports

To access a copy of Appendix A for your IIPP select "Assessment Report IIP Appendix A"

To access a copy of EAP select "Assessment Report EAP"
