Submitting the Application and Fee

Fill out all required fields on the application

Click on Submit Payment

Fill out BearPass (M Number), Full Name, and E-Mail

Click on Add to cart

Click on Continue

Sign in or create a new account to submit payment

Click on Sign in

Set up a new payment method or use an existing one

Acknowledge the terms and conditions and click Continue

Depending on your payment method, you might see a page that details your payment processing fees. Click Continue at the bottom of the page to checkout.

Click on Pay $43 (total will vary based on payment method and fees).

Back on the application page, enter the payment confirmation number and click on Next

You will see a confirmation screen after paying; take note of the confirmation/receipt number. You will need it to submit your application.

Type your first and last name exactly as shown above the name fields

Click on Sign Electronically

The submission confirmation page will display. Click on View Form PDF to download a copy of the application.

Congratulations! Your application is complete. You will receive a submission confirmation email as well as a payment confirmation email; please keep these for your records.
