The Self-Check-In can do many things, such as: (1) CONFIRM your visit, (2) tell us that you've ARRIVED, & (3) can speed up your visit!
Before your visit, you will get a TEXT or EMAIL to do your Self Check-In.

You can choose to request to RESCHEDULE or FILL OUT YOUR REQUIRED FORMS right away to save time on your visit day.

Then, Type your Last Name and Date of Birth, then click Submit

But before you start, we want to verify who you are. Pick an option.
Most of you will choose the option with the RED arrow.

Finish ALL of the Self Check-In STEPS including the Visit Forms.
Visit Forms give us permission to treat you.

Check your Patient Info and click Continue.

Continue to check the rest of your Patient Info and click Confirm & Submit.

If using insurance for your visit, add it here <OR> skip it, if your are Self-Pay by clicking Confirm & Submit.

Upload Insurance card images (BACK and FRONT), if needed and Click on Add

Once finished, click on Confirm & Submit

Read through the Visit Forms (e.g, eConsents, screenings, etc.) and enter your Full name in the "Your Signature" field.

Click on Sign & Submit

Add any medications that you are taking on a regular basis, if needed

Continue to add medications, if needed by choosing from a list.

Complete the details for each medication and then Click on Add

Once you Click on Confirm & Submit, you will see the medication(s) listed

You're all done! You will receive a notification stating "Thanks, you're all set!"
