How to Access Your IIPP/EAP Assessment Form

Log in to EHSA using your single sign-on credentials.

  1. 1. Click on EHS Assessment

  2. 2. For the assessments to appear you may have to click on the down arrow near the "User/PI" field.

  4. 4. Find the IIPP/EAP assessment you wish to fill out and click on "Edit Assessment" button.
