Select Form Template Library

Select the name of your desired Form Template

Select Add Item to add a new Form Item to the Template

<p>Note: Removing Form Items may negatively affect any Analytics Dashboards that exist in your account. Please check with any other Administrators before doing so.</p>

Select the Trash Can Icon to remove a Form Item

Select the text in Form Sections to edit the titles

Select the text of any Form Item to edit the question
