Reporting Behavior Requiring Retaking
1. From the Profile Compact Cases tab create the Violation Report Requiring Retaking

2. Click create.

3. To report behavior requiring retaking, chose 'yes.'

4. Proceed to Current Residence.

5. Select 'No' if the address is current. Select 'Yes' and updated the address if needed.

7. Proceed to Current Employment.

8. Select 'No' if the employment is current. Select 'Yes' and update the employment if needed.

10. Proceed to Violation Information

11. Click to Add a New Violation to the Violation Report

12. Select Behavior Requiring Retaking from Select the type of violation:

13. Date entered should be the date it was determined retaking should occur. Describe in detail how all the incentive and/or corrective selected above addressed or failed to address the behavior.

14. Click on ▶ Attachments

15. Click 'Add' to attach documentation.

16. Provide all supporting documentation in as many attachments as needed. At a minimum provide what your court or parole board expects to formally revoke community supervision and incarcerate the individual.

17. Click on ▶ Probable Cause Information

18. Attach the waiver of his/her right to a PC hearing, hearing report (if a hearing was conducted) or note probable cause for the violation has not been established.
PC documentation can also be added later via Addendum to Violation Report.

19. Review Violation Overview, then 'Save & Continue' to proceed.

20. Proceed to Location/Availability.

21. Provide availability and location information. It is best practice to use the Violation Report Requiring Retaking once the individual is 'available.'
Pending felony/violent crime charge? Use a Progress Report.

22. Provide additional information or attachments related to the violation(s.)

23. Proceed to 'Recommendations.'

24. Provide any additional details confirming need for nationwide warrant for retaking.

25. Be sure to preview the Violation Report to view how it will be saved and display in ICOTS. When completed, click 'Submit.'
